Key Responsibilities

  • Provide comprehensive primary care services across the lifespan (pediatrics, adults, and geriatrics)
  • Perform thorough patient assessments, including medical histories, physical exams, and preventive screenings
  • Diagnose and manage acute and chronic conditions such as diabetes, hypertension, asthma, and other common illnesses
  • Develop and implement individualized treatment plans in collaboration with patients and care teams
  • Prescribe medications and therapies in accordance with New York State regulations
  • Deliver patient education focused on disease prevention, health maintenance, and lifestyle modifications
  • Maintain accurate, timely, and compliant electronic medical records (EMR) documentation
  • Coordinate care with specialists, behavioral health providers, and community resources
  • Participate in quality improvement initiatives and adhere to FQHC clinical standards and guidelines

Qualifications

  • Active and unrestricted (LPN) Nurse Practitioner license in the State of New York
  • DEA license (or eligibility to obtain)
  • Experience in Family Medicine, Primary Care, or Community Health strongly preferred
  • Prior FQHC or underserved population experience is a plus
  • Strong clinical judgment and ability to work independently within a collaborative team
  • Bilingual (Spanish or other languages) is a plus but not required
